Add "Deactivate Account" instructions to Monthly class email

TAMS can send an email to TAs that lists classes. Add instructions for deactivating account (which is on each person's unique URL page)

This email doesn't send automatically and we haven't used it in a while. It might be more useful when we hold more classes more regularly
