support absolute and relative migration paths

Check for _rewrite in the migration path setting, if found just use basename (toss out bad parts that might fetch settings from another database), otherwise path is relative so construct path normally. Absolute paths e.g. /medic/_design/medic/_rewrite/migrate (not recommended) will likely continue to work fine.
