SMDB Clean Up Tasks

Below contains a list of minor fixes we would like to make to the SMDB in the Jan Release.

1. Incentive Bubble Fields - On Participants List, add 2 new bubble fields. Place under 'Study Activity Completion' as the last 2 bubble fields in that section

• 'BL Payment Issued' (130371375.266600170.648936790)
• 'BL Payment Ref' (130371375.266600170.648228701)

4. Campaign Type Bubble Field - On Participants List, move 'Campaign' bubble field from 'Enrollment Details' to 'De-identified Data' section. Place after 'IHCS member status'.

5. Physical Address Labels - On Participant Details Page, we would like to update the 'Physical Address' section so that the "(if different from mailing address)" text is taken off of the individual field labels and placed at the top in the 'Physical Address' header (similar to Alternate Address). There is some concern that CSC agents may only input a partial address in Phys Address if they see it like how it currently is. (e.g. they may think they only need to input line 1 phys address, if the rest of the address fields match the mailing address)
