Add a Review table of contents

The reviews now all follow this structure:

1. Declaration
2. Stage
3. Risks
4. Summary
5. Rating
6. Informational
7. Protocol Analysis
8. Dependencies
9. Governance
10. Contracts & Permissions

We should have a table of content that is always available (not matter which part of the review I'm currently reading), i.e. that can be expanded as a sidebar or just a pop up or so
