Rewrite the DefaultPublicationResolver

The DefaultPublicationResolver now only uses the Web.Config setting to resolve the publication Id, but for older version we can use the dynamic conf to resolve the publicationId and now with CIL we can use the topman services to resolve the mappings.

Hi @ThijsBorst, it is quite embarrassing to reply to an issue that's years old, but I am busy cleaning up to prepare for a new release.
We won't do this in DD4T, it is part of Dyndle (see www.dyndle.com).

I will close this issue in a few weeks.
