Support multiple backend instances

I see a fork from @polar with support for multiple backend instances. It needs tests and probably quite a bit of cleanup to be merged. Putting it out here so that other people aren't restarting the implementation from scratch.

Works for me!

Although, it probably depends on the size of your application, but I'm
hitting the 512Mb limit, or I should a say, message, at 3 backends per
dyno.

We've had the same problem with Unicorn, fyi. But doesn't make the code useless :)
