Report incident/pet concern UX improvements

## Basic Information
Name and email fields should be hidden.  They implicitly come from the user that hits the button.

"Register Pet Concern" should use exactly the same component as "Report Incident". i.e.,
"Vet Name" is not needed.
"Date & Time" should come from the visit, as it does for incidents.
