US-0304: Diff-based transformations only

**As a user,** I want all edits from agents delivered as diffs, **so that** the editor never destructively rewrites my document.

Agents cannot overwrite the full document without a diff.
Patch apply supports accept/reject.
Diff view shows minimal deltas.
