Changing graphics settings breaks controls

If you change graphics settings (resolution, shadows etc) then it goes fullscreen and F5 no longer works to go into windowed and the mouse cursor also gets stuck.

Haven't been able to reproduce this, will find out where it came from and get more details

I think the game might be loading D3D9.dll in some other way as well as the normal way. Will need to investigate.
