Maybe: Split citation input into multiple fields

Ideally, testimony supports should store separate author, title, date, and URL values, allowing us to join them together into a unified citation format.

Commits a27f260 and ac8ef3e are relevant here. Previous versions of the Testimony form sent separate fields for author, publication, title, etc, which were concatenated in insert.php. The form logic now sends a single $citation value.

A revision of this form should go back to sending each element individually, and the database should be updated with a citations table.
