Provide error correction on install

This might become a collection of issues, but need to provide better direction and error correction on install. See #108.

Some ideas:

• Add first role on module setup
• Add error message and redirect to setup if no roles exist
• More useful error message/error prevention (e.g. when adding a user to a team before there is a role)
• Better nomenclature. Role is already used by the core software and it is confusing to have another thing called role in the team context. Responsibility? Position? Duty? Post? Assignment?

Cleanup:
There are some weird artifacts on the update team form that need to be cleaned up

• -correct placeholder for role drop down
• -remove erroneously echoed html (see here)
