review/update setup and getting started instructions

• Rtools no longer needs to be added to path by the user;
• clarify instructions for when to use .Renviron for GITHUB_PAT and when to use git cred store;
• ensure steps for adding GITHUB_PAT occur after installing R (and packages), for when using package functions to edit .Renviron or adding git creds;
