Account for lack of SET-OF columns #50
Description
Right now odkmeta assumes that repeat groups have associated SET-OF columns. However, SET-OF columns may be removed from ODK Briefcase .csv exports. We should use something other than SET-OF columns to order repeat group variables after merging (perhaps the closest previous non-repeat field). We should also only attempt to drop SET-OF columns if they exist.
