Upload file question type for Questionnaire

Ah. So you are asking for a new question type that would require a user to upload a file.
It is possible, but only by coding a new question type.

Dear @anna-sparkfore the Pull request is now #681.
I believe it looks near to being accepted but that is ultimately the decision of @mchurchward.
