Example of create custom Widget on Survey

Hi Mike,

Thanks a lot for create this library.
i have read your documentation that say "Customize it by creating your own form widgets from scratch using the builder method."
and i am interested on this "Builder" thing, but i don't understand how to use it.

do you mind give some simple example how to do it , like create custom widget on the Survey
