I would prefer not to add the complete verbatim survey responses, because there's a lot of extraneous medical/identifying information in the surveys, especially when a parent describes where and who is performing therapy services. So for some answers we'll condense the responses into bullet points.

• When do they wear hearing aids

Communication:

• Age started therapy services
• FM system
• Services summary. This would condense description into bullet points, e.g., "School speech therapy; speech therapy outside of school; DHH instruction.")
• Communication Method: Auditory verbal or ASL or Total communication
