Show Missing Responder Items

Currently the responder items grid only shows items that have been created

Actually there should be all responder items possible shown here (I think)

We could either

* automatically back fill any missing itemtypes and create the responder items before we render teh responder item grid
* extend the grid with psueodo item types

I'm thinking the backfill option is probably best
