Split collateral management by Index symbol #215
Description
Currently all collateral is managed by Chain x Address, however this does not reflect how collateral is stored in OTCCustody, where each instane of OTCIndex has its own tenancy. We shall split by Chain x Address x Index Symbol so that user account has separate collateral position for each Index.
