Multiple checkboxes not working

Multiple checkboxes in a single passage - used to register each with a boolean response, now only registers one 'true'?

Not sure how this changed with the tweaks, but will fix.

Form fields seem to be unaffected.

Just tried to recreate this a second time and it's..... fine???? Hmmmmmmm. Will keep an eye and revisit
