Problem

We would like to process an Entity Name change request from a user. The Entity Name did not automatically validate like it was supposed to because our SAM.Gov UEI validation is temporarily down.

Solution

Complete the Entity Name change request using the process for EINs/UEIs outlined in the internal guidance document (v20250715):

Steps:

• Update the Entity Name in the backend system
• Notify the user
• Document the workflow for future reference

We'll know we're done when

• The Entity name has been successfully updated.
• The user has received confirmation
• We’ve noted any gaps, edge cases, or process improvement areas

Checklist

• Verify the request came from an authorized entity representative
• Confirm the new Entity Name is valid
• Review the form submission and verify affected report ID
• Update the Entity Name in the backend system
• Confirm the report ID reflects the change
• Notify the user of successful update
• Flag follow-up needs, if applicable
