Add ERC-8004 chain name mapping to web UI #11
Description
Add human-readable chain names for all 16 supported ERC-8004 networks:
- CHAIN_NAMES mapping (ETHEREUM_MAINNET → Ethereum, etc.)
- Verdict card shows chain name (e.g. On-chain · Verified · Base)
- Detail section shows chain name instead of alias
