Decoders and protocols fail silently on exception

Trickle exceptions down and handle them properly.

This is not an active bug, the decoders and protocols are pretty much 0 or 1 at the moment and those published work, but it would be nice for debugging purpose?

How do you feel about visionmedia/debug? I tend to use it in all my projects.
