Replace current 'database type' with two distinct variables #281

Expected Behavior
Suggestion to replace the current database type by two distinct fields - both with multiple values allowed per entry:
"Data source type" with possible values 'electronic health/medical. records' , 'claims', 'pharmacy dispensation', 'administrative registries', 'clinical registries', 'biobanks'
and
"Healthcare setting" with possible values "outpatient care - community health care','outpatient care - specialist level', 'inpatient care', 'emergency care'
