Create v0.4 release

Issue to manage remaining tasks for v0.3 release.

• - Go through issues and tag commits to the open issues, close as completed
• - Draft release notes
• - Test release package with partial run (firm synthesis plus one market through to shipments table)

Several markets didn't complete in the mode choice to PMG section,, possibly related to changes to incorporate the calibration code or the transition to the new PMG code. Need to add a little more detail to logging to catch errors at that point and rerun those markets
