Add distinct Learning Outcome field for auto quiz

To generate Learning Outcome based quizzes we need lesson ID and LO tagged separately. Then students can get quizzes based on that.

Also teachers can select lesson specific questions and modify for their own quiz to match their edited CALI lesson.
