Define and use own theme functions for formatter

Currently, user-profile-item/category is used, that should be replaced with custom theme functions.

http://drupal.org/node/951282#comment-3683048 does the same, it should be possible to copy the hook_theme() definitions, theme functions and necessary css definitions and just rename it.
