Topic navigation: standardize click-through behavior

Summary

Standardize navigation so Topics are the primary entry point and meetings are secondary projections.

• Define primary action per card/row (Topic detail).
• Secondary links to meeting pages are contextual and labeled as projections.

Implemented

Meeting show page: "Issues in This Meeting" section

Added a new section after Documents on meeting pages that presents topic cards split into two subsections:

• Ongoing — topics that have appeared across multiple meetings, with intro text: "These issues have come up across multiple meetings. Click any for the full picture."
• New This Meeting — topics appearing for the first time, with intro text: "These issues came up for the first time."

Uses the same _topic_card partial as the homepage and topics index, so residents see a consistent card format across the site. Only approved topics are shown; the section is hidden entirely when no approved topics exist.

The controller (MeetingsController#show) loads approved topics via the meeting's agenda items, eager-loads briefings and appearances, and partitions them by appearance count.

Homepage meeting row topic pills: filtered by importance

Changed from showing the first 3 topics with "+X more" overflow to filtering by resident_impact_score >= 2. No count cap, no overflow indicator, no "No topics yet" fallback. This keeps the pill row focused on issues that matter to residents.

What was intentionally NOT changed

• Topic summary card headers on meeting pages are not clickable — the AI-generated prose doesn't use canonical topic names, and the interaction wouldn't be discoverable to residents unfamiliar with the concept of "topics."
• Agenda item inline topic tags still link to topic pages (unchanged).
