Footnotes

Footnotes are currently not supported. I haven't had much use for them in fiction, but md2ms should be aware of them, either to properly format them in the docx or to strip them out.

Supporting footnotes is going to be a bit of work. The CMark Markdown parser supports them in html, but they don't seem to translate well to Word. word-rs has the fixtures to handle footnotes, but it'll probably require a bit of regex magic to find the `[^1] token, do a look-behind to find the text that should have the footnote, and then another search through the document to find the footnote itself.
