CI failure modes

Empty artifact cascade: If npm run build fails, dist/ artifact is empty. All downstream jobs fail with 404s/import errors.
Silent schedule disable: Malformed cron in quality.yml silently stops scheduled runs. No alert.
Artifact retention: 1-day retention. Manual retry after 24h finds deleted artifacts.
Finalize consolidation: cp -r ... 2>/dev/null || true silences errors. Failed consolidation goes undetected.
Deploy bypass: workflow_dispatch always deploys regardless of quality status (intentional manual override).
Secret dependency: PUBLIC_CONSULT_API_BASE secret required for deploy build. Deleted secret = silent build failure.
